Health Horizons for people living with disability is a 3–6 month, community-led wellbeing programme designed to improve access to clear, accessible health and wellbeing information for chronically disabled adults (18+) in, or connected to, Wiltshire — including Salisbury, Devizes, and Trowbridge.
The programme responds to ongoing barriers disabled people face when accessing health information, support services, and peer connection opportunities. Through interactive workshops, peer-support sessions, and shared learning, Health Horizons aims to build confidence, knowledge, self-advocacy, and long-term wellbeing within local communities.

Why the Programme Exists
Insights gathered from more than 75 individuals with lived experience, through surveys and focus groups, highlighted a clear need for:
* Practical and accessible health and wellbeing information
* Opportunities to build confidence and self-advocacy
* Stronger peer support and community connection
Many disabled people continue to experience gaps in accessible information, inaccessible systems, and limited opportunities to connect with others who understand their lived experiences.
Health Horizons was created to address these challenges through inclusive, accessible, and community-led approaches.

What the Programme Will Deliver
Health Horizons will:
* Deliver accessible workshops led by facilitators with relevant expertise
* Host peer-led support and discussion sessions
* Raise awareness of the barriers disabled people face when accessing health information
* Highlight community-led solutions and lived-experience insights
* Support participants to co-create a practical resource pack and online hub that will continue beyond the project
Carers, support workers, and personal assistants are welcome to attend alongside participants.
If participants have specific accessibility requirements or need support attending, we encourage them to let us know so we can provide appropriate assistance wherever possible.

## About the Programme
Health Horizons is currently funded as a pilot programme, with the aim of developing a sustainable, community-led model for wider rollout across Wiltshire.
The programme is delivered by Disability Equals Ability (DEA) and funded by the National Lottery, in collaboration with local disability networks, community organisations, healthcare partners, volunteers, and people with lived experience.
